We are in-network for most major insurance providers, and a listing of those providers may be found on our website under the Insurance Page. In order to determine if we are considered as an in-network provider for your particular plan you should check with your insurance company.
As a courtesy to our patients, our office will verify your benefits through your insurance provider on or before your first visit and at plan year end thereafter, however it is your responsibility as a patient to update our office about any changes to your plan and to review your plan summary for Physical Therapy.
We will file up to three insurances for any patient and each will be verified for your benefits and eligibility.
A copay is the payment amount that your insurance plan requires you to pay for each of your visits. You will be asked to pay this amount in the office before the beginning of your visit.
Your copay amount depends on what type of insurance plan you are enrolled in. You can contact your insurance company to determine that amount, or you may ask us when you arrive for your visit. In most situations, your copay is the specialist amount listed on your insurance card.
A deductible is a set payment amount that your insurance company requires you to meet before your co-insurance takes effect. Patients with deductible plans are expected to pay a $40.00 copayment to go towards meeting their deductible. Patients may pay more at the time of service. Please keep in mind, because you are paying towards your annual deductible, there may be a balance on your account.
Co-insurance applies to plans with deductibles. Once you have met your deductible, your insurance company will pay a percentage of the cost of your visits and you will be responsible for covering the remainder. For example, a person with a $1000 deductible and 25% co-insurance would be responsible for 100% of the cost of their visits up to $1000, and 25% of the cost over $1000. Co-insurance amounts vary according to the type of plan you have.

Payment of your deductible and co-pay/co-insurance is required at the time of service. We accept cash, check, and credit cards (Visa, Master Card, Discover, and American Express). We will file Medicare, primary and secondary insurance if all information furnished to us is accurate and up to date. We do file up to three insurances, but should financial hardships arise, we encourage you to contact our office for assistance in the management of your account. Financial arrangements can be made.
Future visits may be scheduled by calling the front desk between our regular business hours of 9am-8pm Tuesdays and Thursdays and 10am-2pm on Saturdays. You may also request appointments by visiting the "appointments" tab on the top of this page. Visits can also be scheduled in person at the beginning or end of your appointment.
You may schedule up to 4 weeks in advance.
We recommend that you schedule as far in advance as your schedule allows. This will make it more likely that you will be able to make appointments at times that are most convenient for you. Your therapist will assist in guiding you on your visits after you see them for the first time. If you are recovering from a surgical procedure, we typically advise patients to schedule twice per week for the first month of recovery prior to seeing their assigned therapist. We have typically found this strategy useful to the patient, given the urgency to get them on a regular weekly schedule as soon as possible. Once the therapist sees you, adjustments to your plan of care will be made accordingly. We also encourage patients to remain proactive with tracking their own schedule of visits, and recommend they do not wait until the last session to speak with their therapist about additional sessions, as it can be at times challenging to reschedule seamlessly. If patients are always scheduled out up to four weeks in advance, this will typically avoid them falling off the schedule prior to them actually needing to.
The waitlist is our way of keeping track of patients who need to make appointments, but who cannot find times that will work for them. If you need to schedule an appointment and find that your therapist’s schedule is full for the days or times that you are available we can add you to our waitlist. We will keep track of changes in the therapist’s schedule and let you know if an appointment becomes available.
If there is a change to your appointment time we will contact you by phone to let you know and to confirm that your schedule still allows you to make it to your appointment
Appointment lengths vary. Initial evaluations last an hour, giving your therapist time to assess your condition and develop a routine of care. When you come in for follow up appointments, you should expect to be in the office for about an hour.
Based on New York State regulations, a prescription is not required to be seen by a physical therapist. However, some insurances do require prescriptions before they will process and pay the claims from the visits. Most insurance plans do not have this requirement, but you should check with your insurance carrier to determine which applies to your particular plan. We recommend that you have a referral with you at your initial evaluation.

While we believe that it is in a patient’s best interest to remain with one therapist for the duration of their treatment, we are more than happy to accommodate requests for therapist changes.
We understand that things can change, and are happy to cancel or reschedule an appointment if we are notified more than 24 hours in advance. Please be aware that appointments cancelled less than 24 hours prior to the designated time, or failure to arrive for a scheduled appointment, may result in a $25.00 missed appointment fee. This fee may be waived at the discretion of the management depending on your past history and current circumstances.
If you are running late, please let us know as soon as possible so that we can notify your therapist. Patients who arrive more than 15 minutes late may forfeit their appointment and be subject to a $25.00 missed appointment fee, subject to the owner’s discretion.
Your visit will likely include some physical activities, so it is best to be dressed comfortably. Most of our patients choose to wear workout clothing and athletic shoes. It goes without saying, the body part that needs to be treated needs to be easily accessible by the therapist. For example, if you are experiencing knee pain, you may want to avoid wearing pants that cannot rise above your calf to allow the therapist to assess the area. You are welcome to bring a pair of shorts which will allow easy access to the knee for the therapist to diagnose the condition.
At your initial evaluation your therapist will provide you with a thorough interview and assessment to better understand your condition, the reason you are seeking physical therapy services, and to develop a comprehensive plan of care to meet your rehabilitation goals. The prognosis of your condition can also be determined at your first session. You will be given an assessment of your condition and what the therapist's treatment plan for your smooth recovery.
Ongoing appreciation, assessment, and specific, tailored treatments will be provided at each regular visit, with the goal of moving your condition forward towards your goal attainment.
You can maximize your therapy by taking an active role in your plan of care. Listening to your therapist, complying with your plan of care, adhering to your home exercise plan, and respecting your symptom behavior are all vital to a successful recovery.

Prevention The practice of sports physical therapy includes those interventions that assist the athletically active individual in preventing injuries and then, if an injury occurs, continuing their pursuits with minimal disruptions. Areas of this practice include pre-participation screening, equipment recommendations, and cardiovascular fitness programs designed to assist in a safe and unremarkable return to activity.
Evaluation Sports physical therapists are highly skilled in evaluation of active and chronic injuries. A hallmark of this practice is the assessment of surgical and non-surgical neuromusculoskeletal injuries. Included in this is the functional assessment of the individual post-injury to assist in a safe return to activity.
Treatment & Rehabilitation Treatment and rehabilitation of neuromusculoskeletal injuries is at the center of sports physical therapy practice. In a team approach with physicians, the sports physical therapist is able to design and implement programs for the management of these disorders in clients of all ages and physical abilities including those with physical disabilities as well as elite athletes.
Performance Enhancement Sports physical therapists are able to assist athletically active individuals improve their performance in a variety of ways. The foundation of performance enhancement is provided by a thorough evaluation of the neuromusculoskeletal and the cardiovascularpulmonary systems. The results of this evaluation are compiled to outline strengths and weaknesses in an individual's physical profile. This profile can then be matched to the individual's specific athletic activity. Areas of deficiency in flexibility, muscular strength and endurance, and in the aerobic/anaerobic systems can be outlined in a specific training regime. Research Treatment and rehabilitation are constantly changing in response to our interpretation of the basic and clinical sciences. Education Education of athletes and their parents and coaches about the various preventative and management techniques previously discussed is a service that sports physical therapists provide. An example is if an athlete would like to improve their sprinting velocity, they would engage in a specific type of training customized for their goals and expectations. Performance of this program may facilitate improvement in their velocity during sprinting.
